Range: $62.78 - $78.47 - $94.16The Nurse (RN) - Navigator, Oncology
is a highly skilled nursing professional responsible for optimizing
care coordination, enhancing the patient experience, and improving
clinical outcomes for oncology patients. This role provides
nurse-led navigation throughout the patient journey, from diagnosis
to survivorship. The Nurse (RN) - Navigator performs nursing
assessments, triages patient care needs, and implements care plans
in collaboration with the oncology care team. Key responsibilities
include scheduling appointments, providing logistical support,
acting as a liaison between patients and the medical team,
directing patient inquiries to appropriate staff, and facilitating
the collection of medical records, insurance, and financial
information. This position involves direct patient communication in
person, by phone, and through the patient portal.Job Requirements,
Prerequisites And Essential FunctionsEssential Functions and
Responsibilities:Individual
- Educates patients on the purpose of visits, treatment plans,
and available clinical trials.
- Assesses patient education needs and provides disease-specific
information, symptom management guidance, and referrals to
supportive care programs.
- Maintains clinical knowledge of the breast cancer disease
process, clinical trial information, symptom management, and
appropriate cancer care services to support patient care.
- Ensures interpreter services are arranged as needed for all
appointments.
- Tracks consultations, new patient visits, and follow-ups to
optimize timely access to care.
- Confirms patient appointments in advance and ensures
demographic details are accurately recorded.
- Oversees the scheduling and rescheduling of patient
consultations and follow-ups.
- Facilitates access to community resources, transportation,
insurance support, and advance directives.Team
- Acts as the primary liaison between patients, caregivers, and
the oncology care team, addressing mental, emotional, and physical
needs.
- Promotes ongoing communication and collaboration with clinical
leadership, surgeons, and staff to maintain patient safety and
adherence to clinic protocols.
- Ensures all documentation is accurate, updated, and
audit-ready.
- Notifies the surgical team of any status changes or obstacles
to treatment compliance.
- Communicates with internal and external referrals to ensure
prompt patient access.
- Assists in maintaining safety standards within the clinic.
- Updates clinical leadership on changes affecting the surgical
schedule.
- Manages group processes and builds effective working
relationships within the care team and with external
stakeholders.Practice
- Identifies and resolves barriers to care, ensuring at-risk
patients receive necessary services.
- Takes initiative in identifying and implementing process
improvements for patient navigation.
- Participates in clinic meetings and contributes to workflow
improvements.

- Implements professional and community-based education programs

- Utilizes strong problem-solving, decision-making, and

records (EHR) systems for documentation and coordination.
- Applies knowledge of medical terminology to communicate
effectively with healthcare providers and patients.
- Organized and detail-oriented with strong follow-through
abilities to ensure continuity of care.
- Exhibits strong communication and interpersonal skills to
foster a patient-centered approach to care.

- Knowledge in adult learning styles and various teaching
methods: ability to deliver educational programs to staff and

- Experience with Epic EMR

interdisciplinary health care team and provide direction or

responsibility to ensure quality care
- Ability to develop materials involving graphics and
tablesQualificationsEducation:
- Bachelor's Degree of Science in Nursing from an accredited
college/university
- MS or MSN preferredLicense & Certifications
- Current Registered Nurse California license.
- OCN - Oncology Nurse Certification preferred
- AOCNS - Advanced Oncology Certified Clinical Nurse Specialist
preferred
- BLS Certification (American Heart Association)
required.Experience
- Minimum of 5 years of nursing experience
- Minimum 2-3 years of experience in oncology
- Experience with nurse navigation and/or ambulatory oncology
