URGENT HIRING FOR REGISTERED NURSE CASE MANAGER Job Locations : Santa Rosa, CA Job Duration: 13 weeks GTDW : 40hrs Shift : Day 5x8-Hour (08:00 - 16:30) Responsibilities : Coordinate and manage patient care plans across the continuum of care. Conduct initial and ongoing assessments to determine patient needs and appropriate services. Collaborate with physicians, nurses, social workers, and other healthcare professionals to ensure quality and timely care. Monitor patient progress, evaluate outcomes, and adjust care plans as needed. Provide education to patients and families regarding diagnoses, treatment options, and community resources. Ensure safe transitions of care, including discharge planning and follow-up coordination. Advocate for patient needs while ensuring cost-effective care delivery. Document all case management activities accurately and in compliance with regulatory and organizational standards. Review clinical documentation to determine medical necessity for services or level of care. Identify barriers to care and work with stakeholders to resolve them.
