Patient Care Coordinator Experience: 1–2 years in care

License: Active State or Compact RN License Certifications: BLS –

in discharge planning and interdisciplinary collaboration
Description: The Patient Care Coordinator RN oversees the continuum
of care for patients, ensuring quality and efficiency throughout
hospitalization and discharge. Responsibilities include
