Job Description Job Description Travel RN Case Manager – 13 Week Contract Greenbrae, CA 94904 (On site) 13-Week Travel Assignment Day Shift Monday through Friday - 0730 to 1600 We are seeking an experienced Travel RN Case Manager for a 13-week contract assignment in beautiful Greenbrae, California . This is an excellent opportunity to expand your experience while working in a dynamic healthcare setting. Position Overview: The RN Case Manager will coordinate patient care across the continuum, ensuring appropriate utilization of services and optimal patient outcomes. You will collaborate closely with physicians, nurses, social workers, and insurance providers. Key Responsibilities: Conduct comprehensive patient initial assessments Perform utilization review and ensure medical necessity Apply InterQual criteria for level of care determinations Coordinate discharge planning and transitions of care Document thoroughly in EPIC EMR Collaborate with interdisciplinary teams to optimize patient outcomes ?
