Sutter Care at Home is the home health care and hospice affiliate

The Infusion Team Nurse provides skilled nursing services specific
to the needs of infusion patients, including instruction and
evaluation in the home, to a designated group of patients under
physician direction. The Infusion Team Nurse is accountable to the
Infusion Team Leader for ensuring that the plan of care is
implemented following professional standards of care and agency
specific protocols.
Primary intended coverage area: North and Central San Mateo County
- Daly City to Redwood City
Education:
RN graduate from an accredited curriculum. Bachelor's degree in
nursing or health related field preferred.
Licensure:
Current California RN license; valid California drivers license
with proof of current auto insurance; current BCLS/CPR
certification. CRNI (Certified Registered Nurse in Infusion)
preferred.
Experience:
One year experience in an acute care setting required; experience
with infusion therapy is strongly preferred. One year of experience
in a home health position preferred, preferably in infusion
therapy. Direct patient care experience within the past five years
required.
Knowledge:
Basic knowledge of infusion services and infusion clinical
standards of care and practice. Must have the ability to function
independently and as a member of an interdisciplinary team; ability
to exercise independent, sound nursing judgment in planning and
providing patient care. Exhibits knowledge of patho-physiology and
accepted treatment protocols for infusion home care diagnoses.
Knowledge of safety and infection control principles in
relationship to infusion therapy. Knowledge of state and federal
home care regulatory guidelines preferred.
Special Skills/Equipment:
Infusion, phlebotomy/venipuncture experienced and trained.
Chemotherapy trained is preferred. Current California RN license.
Valid California driver's license and access to a reliable
automobile with proof of liability insurance. Demonstrated clinical
assessment skills in relationship to infusion therapy. Ability to
recognize the special needs of infusion patients and others in the
home. Ability to communicate with patients and significant others
in an effective, mature, caring manner. Ability to problem solve
during after hours on-call coverage via phone triage. Ability to
determine via phone triage when a nurse needs to make an
unscheduled home visit to a patient. Ability to maintain harmonious
constructive relationships with internal and external customers.
Demonstrated effective written and verbal communication skills.
Must be able to recognize unsafe or emergency situations and act
appropriately.
Must be able to meet company productivity standards as indicated by
organization and manager. Must be able to handle sensitive issues,
conflict with or among others, respectively direct and reinforce
staff efforts; effectively plan and organize and prioritize work,
think critically to both effectively plan and organize department
operations consistent with Sutter Care at Home's strategic and
operating objectives and to effectively solve unique problems as
they arise or identify when to consult supervisor. Maintain a
positive and professional image and demonstrate self-directed
learning to meet professional development and/or license
requirements. Responsible for maintaining all required licensure
and certifications. Must have reliable transportation.
