The Intake Coordinator works closely with the clinical outreach nurses, hospital case managers, physician offices and other community referral agencies.*Responsibilities: **50% Administration and Operations* - Provide information regarding Mission Hospice & Home Care services to referral sources including physicians, case managers, patients, family, and community agencies. - Screen and process referrals for admission to Mission Hospice & Home Care programs to include coordinating admission visit, confirming physician, entering information into EMR. - Confirm and track required documentation and files for admission - Facilitate daily intake meetings reviewing planned admissions, staffing, documentation needs to ensure effective onboarding of patients. - Collaborative with administrative department to ensure billing, documentation, patient health information are addressed according to current intake procedures. - Analyze and contribute to development of intake procedures to ensure adaptability to current regulations and efficiency. - Collaborate with hospital case managers to facilitate and coordinate discharge plans for patients coming onto service with Mission Hospice & Home Care. - Communicate with Outreach Nurses, IDG, triage LVNs, and scheduler to assure smooth transition into Mission Hospice & Home Care programs. - Maintain customer relations database to include all current and potential referral sources in EMR and other documentation databases. - Collaborate with other members of the clinical team to enable a smooth transition for patients and families enrolling in either the hospice or home care programs. - Other duties as assigned*30% Documentation* - Effectively utilizes EMR, communication tools (email, tiger text, phone) to update team, IDG, referral sources, facilities, community partners on current status of referral and/or current patients. - Develop and maintain processes, data management and analysis to accurately represent the department and identify opportunities for development. - Assists the Chief Care Integration Officer in the preparation of an annual budget for the intake department and monitors allocation of resources according to budgetary limitations.*20% Compliance & Risk Management* - Assures proper entry of initial patient documentation in compliance with state, federal, and Joint Commission standards. - Remains current on matters of agency policies/procedures, relevant clinical knowledge through continuing education, professional organizations, etc. - Participates in providing, reviewing and revising written social service policies and procedures - Actively participates in quality assessment performance improvement teams and activities*Qualifications**: * - High School Diploma - Minimum two years experience in healthcare setting - Valid California Driver's License and an automobile that is insured in accordance with state requirements - Computer proficiency in Microsoft Office and willingness to learn new software as needed - Team player with excellent organizational, communication, and problem-solving skills*Physical abilities/requirements: * - Ability to safely lift/transfer 10 lbs with or without assisted devices (patients, supplies, etc) - Ability to push, pivot, pull, and reposition 10 lbs with or without assisted devices - Ability to bend, squat, stoop, crouch, kneel, reach, climb, and rotate - Manual dexterity (grip/grasp/pinch) - Ability to stand for prolonged periods of time - Ability to walk for prolonged periods of time - Ability to drive for prolonged periods of time - Visual and hearing acuity with or without assisted devices - Withstand different work environmentso Internal- office noise and interruptions, open workspaceso External- a number of elements including but not limited to: various community & living environments, weather, & travel/traffic conditionsPlease Note: The salary range provided here is only a reference guide, but final salary rate depends on work experience.Job Type: Full-timePay: $25.00 - $37.00 per hourBenefits:* 401(k)* 401(k) matching* Dental insurance* Employee assistance program* Flexible spending account* Health insurance* Health savings account* Life insurance* Paid time off* Pet insurance* Referral program* Retirement plan* Vision insuranceMedical specialties:* Home HealthSchedule:* 8 hour shift* Weekend availabilityCOVID-19 considerations:All staff are required to be fully vaccinated which included Covid-19 booster.Education:* High school or equivalent (Required)Experience:* Healthcare: 2 years (Required)* Computer skills: 1 year (Required)Work Location: One location
